利用規約Terms of Use

1.自己責任での参加について
本プログラムへの参加は、すべてご自身の責任において行っていただきます。体調や安全面に十分ご配慮のうえ、無理のない範囲でご参加ください。

2.業務時間外での参加について
本プログラムは業務時間外に実施されるものであり、勤務時間中の参加はご遠慮ください。参加される際は、始業前・休憩時間中・終業後など、就業規則に則った時間帯でのご利用をお願いいたします。

3.労災適用外について
本プログラムは私的活動として位置づけられており、万が一事故や怪我が発生した場合でも、労災保険の適用対象とはなりません。あらかじめご了承のうえ、ご参加ください。

個人情報保護方針Privacy Policy

株式会社アスポ（以下「当社」という）は、IBMフィットネスセンターGENKIのメンバー（以下「メンバー」とい
う）からお預りする個人情報を厳格に保護し、適切に取扱うことを最重要と考え、個人情報の保護に適用される
法令、ガイドラインおよびその他の規範を遵守するとともに、個人情報保護方針を掲げ、これを確実に実施して
まいります。

1.個人情報の収集・利用目的
メンバーの個人情報は適切な方法で収集するものとし、情報主体（以下「本人」という）への通知または公表し
た目的および範囲に限定して利用することとします。
当社がメンバーの個人情報を収集し、利用する目的は下記のとおりです。
①入会の手続きのため
②当社サービスに関するお問い合わせにお応えするため
③運営に関するご案内、各種キャンペーン・イベント等のダイレクトメールをお送りするため
④お問い合わせ時のご本人確認ならびに内容確認のため
⑤お客様の施設利用時の安全管理のため
⑥アンケート依頼などを含めたマーケティング活動全般のため
⑦顧客動向分析または商品開発等の調査分析のため
⑧個人を特定できない形での統計データの作成、活用、公表のため
⑨防犯のため⑩その他メンバーに通知した目的のため
上記以外の目的で個人情報を収集して利用する際には、法令に定める場合を除いて、あらかじめ本人の同意を得
た上でこれを利用します。また、上記の利用目的を変更する場合には、法令に定める場合を除いて、その利用目
的とお問い合わせ先を明示します。

2.第三者への情報提供
法令に定める場合を除いて、本人の同意を得ずに第三者に個人データを提供することはありません。

3.個人情報の正確性および安全性の確保
個人情報の正確性および安全性を確保するため、個人情報に対する安全対策を実施し、個人情報への不正アクセ
スまたは個人情報の紛失、破壊、改ざん、および漏えい等の予防ならびに是正に努めます。

4.業務委託の見直し・改善
当社は、上記1の利用目的の達成に必要な範囲内において、個人データの取扱いを第三者に委託することがあり
ます。業務委託について、個人データの保護をより確実にするため、見直し・改善を図ります。業務委託契約を
締結する際には、委託先に当社個人情報保護方針の遵守を義務付け、委託先に対する管理・監督を徹底いたしま
す。

5.個人情報保護管理体制及び全従業者への周知・徹底
適切に個人情報保護に関する活動を行える体制を整備し、さらに継続的な改善を実施いたします。また、全従業
者（役員、社員、関係スタッフ）に対して、個人情報保護に関する取組みの周知・徹底を図ります。

6.個人データの開示、訂正、利用停止、第三者提供の停止
当社が保有する個人データの利用目的の通知、開示、訂正・追加・削除、利用停止・消去および第三者提供の停
止を希望される場合には、後記9のメールアドレスまでご連絡ください。請求者がご本人であることを確認させ
ていただいた上で、法令等の定めに基づいて対応させていただきます。

7.法令等の遵守
個人情報の取扱いにおいて、当該個人情報の保護に適用される法令及び関連するガイドライン・その他の規範を
遵守します。

8.コンプライアンス・プログラムの継続的改善
個人情報を保護し、適切に利用させて頂くための個人情報保護に関するコンプライアンス・プログラムを策定し、
これを着実に実施します。さらに、これを維持し、継続的改善を図ります。

Terms of Use

1.Participation at Your Own Risk
Participation in this program is entirely at your own risk. Please be mindful of your physical condition and safety, and take part only within a range that does not cause strain.

2.Participation Outside of Working Hours
This program is conducted outside of working hours, and participation during working hours is not permitted. When taking part, please do so before work, during breaks, or after work, in accordance with workplace regulations.

3. Not Covered by Workers’ Compensation
This program is considered a private activity, and in the event of an accident or injury, workers’ compensation insurance will not apply. Please understand this in advance before participating.

Privacy Policy
ASPO Co., Ltd. (hereinafter referred to as the "Company ") recognizes the importance of personal information
which belongs to our customers in IBM fitness center GENKI. Since IBM entrusted such an importance to us,
company swears to manage and handle appropriately by complying with the laws, guidelines and regulations
that apply to personal information protection.

Acquisition of personal information and Purpose of using personal information
Our company will obtain customer’s personal information by fair and appropriate means and utilize it within
customer’s agreements for the purpose of achieving the following purposes.
① Registration for our membership.
② Response to inquiries and requests regarding our services.
③ Notification of services, campaigns, events, etc., offered by our company.
④ Customer identification for inquiries and its details.
⑤ Safety operation and management for users of our facility.
⑥ Implementation of our marketing activities.
⑦ Analysis for customer tendency and product development.
⑧ Practical use and official announcement of our statistic date with veiling personal information.
⑨ Crime prevention
⑩ Other purposes announced to members.
Except for purposes mentioned above, our company will never obtain and use personal information unless
required by laws and regulations or by the consent of the customer. If company needs to revise the purposes
mentioned above, details including contact of inquiry will be specified.
Disclosure and provision of information to a third party
Our company will not disclose or provide personal information to third parties unless required by laws and
regulations.
Management of personal information
Our company takes utmost care to ensure that we protect personal information from unauthorized access, loss,
destruction, falsification, leakage, etc., by handling our security system and policy properly.
Business entrustment
For achieving purposes mentioned, our company may entrust a part of its managements of personal information
to third parties. In these cases, company will implement all appropriate measures in managing and supervising
such third parties to safeguard the handling of customers’ personal information, including establishing
agreements on the handling of such personal information.
Security system for personal information and Staff training for appropriate operation.
Our company will put system for safeguarding the security of personal information in order and ensures that
the board members, employees, and other staffs are properly trained regarding appropriate handling the system
and operation for the security of information identifying individual customers.
Request about handling of personal information.
For the request of disclosure, correction, addition, deletion, discontinuance or suspension of provision to third
parties, please contact us by email to the address No.9 bellow. The request will be handled based on laws and
regulations, after confirming that the request was submitted by the customer themselves.
Compliance with laws and regulations.
Our company complies with the laws and regulations that apply to personal information protection, and strive
to protect personal information owned by the company.
Ongoing review of the compliance and program.
Our developed compliance policy and program for the protection of personal information will be operated
properly and we will review these compliance policy and program for further improvement.
